Its a National Cash Register Stamping Phone. The clerk would use it to call the back room to look up how much credit you had at the store, and then the machine would be used to stamp a receipt form.

I know this thing started life as a mantel clock, at some point had its clockworks ripped out and was mounted in a wall and stuffed with straw. That would make it an aaaaaaaaaaaaaaaarrrgh.
