When I was in high school, hall passes were not really required because the staff knew every student. But one teacher wanted to discourage the many students who wanted out of class to go to the bathroom, so he made us carry a toilet seat when we asked to go. Fast forward forty years, and most larger high schools require a hall pass for every student who is not in class during class time. Teachers have created large, embarrassing, and difficult hall passes of all kinds to try to limit bathroom breaks to those who really need to go.
