The Lord of the Rings Pin-Up Style Tattoo

I've only read through Fellowship and halfway through Two Towers, so I assume that this scene takes place later in the trilogy. But then why wasn't it featured in the movies?

I am amazed that any woman would give birth . . . twice. I cannot have but the utmost respect and awe for people who have such love and commitment that they would face ". . . Death by torture for each life beneath her breast" (per Kipling) not once, but multiple times.
