It hearkens back to the days when games came without explanation, where you had to stumble around to figure out what's going on, and the simple style masked a devilishly difficult puzzle element. You have to press space bar to select (or do anything in-game).

You're not missing anything though. It's like the original Wolfenstein only way less fun. The "head bob" will make most everyone nauseous after a while. That is why later games had the option to turn it off.
